About the position

The SLH Case Manager RN is responsible for delivering comprehensive case management services to clients with complex health conditions and social challenges. The role focuses on providing timely and intensive case management during the transitional period from hospitalization to community integration, aiming to prevent health deterioration and reduce the need for costly acute care services. The Case Manager RN also works to develop self-management skills in clients to improve their long-term health status.

Responsibilities

  • Conducts discharge planning assessments in a timely and complete manner, considering the age and cultural or religious influences of the patient/family.
  • Coordinates disposition of Medi-Cal, self-pay, and out-of-plan patients, including working with physicians to determine readiness for transfer and securing placement.
  • Encourages patients to develop realistic plans and refers them to appropriate community resources, maintaining knowledge of available resources.
  • Guides patients in understanding their illness and prepares them for discharge by conferring with patients and their families.
  • Provides community resource education and coaching, focusing on individual client self-management principles.
  • Reassesses the client's condition when changes occur and revises the care plan as appropriate, coordinating needed services with local resources.
  • Reviews patients in the caseload for medical necessity and appropriateness of care, coordinating with the interdisciplinary team for discharge planning.
  • Serves as a mentor and educator to the healthcare team, assisting with care coordination and quality improvement activities.
  • Understands insurance and accrediting agency standards to determine criteria for admissions and treatment, coordinating denials with the attending physician.

Requirements

  • Associates degree in a related field; Bachelor's Degree preferred.
  • Two years of related experience in an acute care facility and/or utilization review training.
  • Varied clinical experience or experience in case management/community health preferred.
  • Valid license to practice as a Registered Nurse in the State of California.
  • BLS - Basic Life Support Certification issued by AHA-American Heart Association.
