About the position

The SOAR Coordinator is responsible for providing street outreach to individuals experiencing mental illness and housing crises, with the goal of assisting them in applying for disability benefits through the SSI/SSDI Outreach, Access, and Recovery (SOAR) Program. This role involves engaging with clients, assessing their needs, and connecting them to necessary services, while also providing support to those in hospitals. The position emphasizes building trust and rapport with clients, ensuring they receive the assistance they need to achieve stability and recovery.

Responsibilities

  • Conduct outreach to individuals experiencing mental illness and housing crises in Davidson County.
  • Establish rapport with individuals experiencing homelessness and gather information for disability benefits applications.
  • Utilize the SOAR Nashville process for connecting clients to income and insurance services.
  • Provide ongoing support and referrals to services after clients obtain disability benefits.
  • Complete required documentation in Park Center's Electronic Medical Record and other systems for the disability process.
  • Engage clients in a manner that promotes trust, safety, and autonomy.
  • Interview clients to gather necessary information for housing and services.
  • Assist in linking individuals to resources and provide transportation as needed.
  • Provide in-reach services to individuals in hospitals who may be eligible for SSI/SSDI benefits.
  • Collaborate with social work staff and treating physicians to ensure comprehensive care.
  • Complete screening/intake packets for potential program participants within specified timeframes.
  • Submit comprehensive medical evidence to SSA/DDS and track application progress.

Requirements

  • Bachelor's Degree in Psychology, Sociology, Social Work or related field.
  • At least 2 years of experience serving individuals experiencing mental illness, substance use, or homelessness.
  • Strong time management skills and ability to meet deadlines independently.
  • Proven written and verbal communication skills and computer literacy.
  • Valid driver's license and proof of personal automobile insurance.

Nice-to-haves

  • Experience in community outreach or social services.
  • Knowledge of the SSI/SSDI application process.
  • Familiarity with local resources for individuals experiencing homelessness.
