About the position

The SOAR Specialist (SSI/SSDI Outreach, Access, and Recovery) plays a crucial role in assisting seriously and persistently mentally ill adults by providing goal-oriented activities that help them access necessary care and services. The position focuses on monitoring, clarifying, and assisting clients with personal benefit administration, ensuring they receive the appropriate entitlements and support tailored to their individual needs.

Responsibilities

• Manage a case load of individuals receiving residential, outreach, or monitoring services.
• Assist clients in personal financial administration.
• Complete SSI & SSDI applications using the SOAR model.
• Communicate with local SSA & DDS representatives.
• Track application outcomes using OAT tracking.
• Provide appropriate therapeutic interventions.
• Manage aspects of BPHC/AMHH applications, reassessments, and DMHA audits.
• Assist clients as Authorized Representative for Medicaid benefits applications and maintenance of benefits.
• Complete and monitor transitional loans for CSS clients.
• Complete system updates on client packages including prior authorizations.
• Complete electronic medical record documentation as required.

Requirements

• Bachelor's degree in psychology, social work, or related human services field is required.
• Previous experience working in mental health or related discipline preferred.
• Successful completion of OBHP educational requirements within 30 days of hire date.
• Successful completion of SOAR online training.
