About the position

The Social Service Assistant (SSA) at Briar Place Nursing and Rehabilitation plays a crucial role in supporting residents' psychosocial needs and ensuring their well-being. This position involves a variety of responsibilities, including inventory management, conducting psychosocial groups, monitoring resident behaviors, and facilitating communication between residents and staff. The SSA is integral in maintaining a safe and supportive environment for residents, particularly during smoke breaks and other activities.

Responsibilities

• Inventory all resident's items, including admissions, discharges, room changes, and current residents.
• Document inventoried items on the inventory sheet and in PCC.
• Conduct psychosocial groups and document participation in the groups.
• Monitor, observe, de-escalate, and redirect behaviors during smoke breaks, reporting to PRSC's and PRSD.
• Ensure residents sign and complete the smoking contract.
• Conduct room searches for contraband items.
• Educate residents on facility handbooks and house rules, ensuring they sign them.
• Schedule dental appointments for residents through Senior Dental and notify PRSD of any refusals of care.
• Document time and date for wander guard checks daily and report any changes to PRSD.
• Assist with room changes and notify families accordingly.
• Maintain the token store, updating token cards and points as needed.
• Distribute all mail and packages daily.
• Monitor dining to ensure all trays are passed during meal times.
• Communicate changes in resident condition, functioning, and behavior to PRSD.
• Perform any other job duties as assigned by department managers.

Requirements

• Certified Nursing Assistant (CNA) certification is required.
• Entry-level position, suitable for individuals with minimal experience.
