The Social Service Clinician in the Adolescent Unit at Butler Hospital is responsible for providing comprehensive psychosocial assessments and discharge planning for patients and their families. They will gather and interpret psychosocial data, demonstrating awareness of age-specific factors such as signs of abuse and educational needs. The clinician will also be responsible for completing discharge planning and intervention plans within the required timeframes, ensuring that all documentation is accurate and timely. The clinician will participate in team meetings, contribute to treatment planning, and advocate for patients and families within the hospital and community. They will provide education and support to patients and families regarding the patient's illness, treatment options, and community resources. Additionally, the clinician will supervise and consult with non-licensed staff and provide training and in-service education to social services staff and other health professionals.
