The Social Services Assistant at Windsor Convalescent Center of North Long Beach plays a crucial role in supporting residents and their families during their transition into the facility. This position focuses on assisting individuals in adjusting to their new environment, managing their care plans, and ensuring compliance with federal and state regulations. Responsibilities

  • Assist residents and families in adjusting to placement and illness.
  • Support residents' and families' wishes regarding treatment.
  • Participate in resident and family council meetings as requested.
  • Assist with admissions and discharge planning services.
  • Maintain confidentiality and discretion in all interactions.

Requirements

  • High school diploma or GED required.
  • Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
  • Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines pertaining to long-term care.
  • Ability to organize, prioritize, and meet deadlines.

Nice-to-haves

  • Experience with the geriatric population.
  • Associate's or Bachelor's degree in a relevant field.
  • Creative problem-solving skills.
  • Strong communication skills.
  • Experience with PointClickCare (PCC).
