About the position

The Social Services Assistant/Case Management Director at Divine Rehabilitation and Nursing is a dual role focused on providing compassionate support and management of social services programs within the facility. The position involves assisting the Social Services Director in various administrative and clinical tasks, ensuring effective care planning, and collaborating with interdisciplinary teams to enhance resident care.

Responsibilities

  • Assist the Social Services Director in planning, developing, organizing, implementing, evaluating, and directing social services programs.
  • Complete BIMS, PHQ9, PASS RR, LOC, MDS and other assessments.
  • Manage, follow up, and maintain the facility's concern log.
  • Meet with administration, medical and nursing staff, and other related departments in planning social services as directed.
  • Participate in discharge planning, development, and implementation of care plans and resident assessments.
  • Encourage residents/responsible parties to participate in the development of their care plan and invite them to care plan meetings accordingly.
  • Assist residents in obtaining transportation to medical appointments, upon discharge, etc.
  • Accurately and completely document social service actions and interactions in each resident's medical record.
  • Coordinate Social Services activities with other departments as needed.
  • Support/Cover the Social Services Director.
  • Send insurance updates to insurance plans to determine length of stay and clinical, social, and physical needs.
  • Collaborate with other departments in obtaining and understanding the resident's medical information for review.
  • Participate in the facility's Manager on Duty program.
  • Other duties as assigned.

Requirements

  • 1 year of experience in Social Services or Case Management in long-term care (Required).
  • Strong time management and organizational skills.
  • Attention to detail.
  • Ability to work with a diverse resident population.
  • Ability to effectively work with interdisciplinary team and facility staff.
  • Excellent customer service skills.
  • Strong understanding of Microsoft Office Suite, including Outlook, Teams, Word, and Excel.
