About the position

Lemay Avenue Health and Rehab, located at 4824 South Lemay Avenue, Fort Collins, CO, is a skilled nursing facility that provides a comprehensive range of services including 24-hour skilled nursing care, physical therapy, occupational therapy, respiratory therapy, speech-language therapy, secure memory units, and both group and individual activities. We are currently seeking a dedicated Social Services Assistant to join our team on a part-time basis, working less than 24 hours per week, including weekends as needed. The starting wage for this position ranges from $15.00 to $16.00 per hour, depending on experience. As a Social Services Assistant, you will play a crucial role in supporting the rehab unit by assisting with admissions and discharges. You will advocate for residents and their families during their stay at our facility, ensuring they receive the necessary support and resources. Collaboration with Social Workers and other departments within the facility, as well as community resources, is essential to provide comprehensive care. You will assist residents in exercising and protecting their rights and contribute to the creation of individualized care plans tailored to each resident's needs. Your responsibilities will also include helping residents and their families navigate resources related to discharge planning and post-discharge support. Timely and accurate documentation and assessments related to cases will be part of your duties, along with overseeing the coordination, management, documentation, and scheduling of ancillary services. Participation in resident council and neighborhood meetings, scheduling Care Conferences with residents, family members, and staff, and assisting staff in managing social and emotional factors, including crisis intervention, are key aspects of this role. Additionally, you will assist with audits and support the Social Services department with various workloads as needed. All duties must be performed in accordance with facility policies and procedures, including adherence to HIPAA requirements, and you will be expected to cooperate with the responsibilities of being a mandated reporter of abuse and/or neglect.

Responsibilities

  • Support the rehab unit and assist with admissions and discharges.
  • Advocate for residents and their families while at the facility.
  • Collaborate with Social Workers and other facility departments as well as community resources.
  • Assist residents in exercising and protecting their rights.

  • Assist residents and their families with resources pertaining to discharge planning and post-discharge planning.
  • Complete timely and accurate case-related documentation and assessments.
  • Oversee coordinating, managing, documentation, and scheduling of ancillary services.
  • Participate in resident council and neighborhood meetings.
  • Assist in scheduling Care Conferences with residents, family members, and staff.
  • Help staff manage social and emotional factors and crisis intervention.
  • Assist with audits.
  • Support the Social Services department with workloads as needed.
  • Perform all other duties as assigned, including administrative assistant and social services duties.
  • Follow and adhere to facility policies and procedures implementing HIPAA requirements.
  • Assist in interdisciplinary meetings.
  • Cooperate with expectations related to being a mandated reporter of abuse and/or neglect.

Requirements

  • Associate's degree in the human services field preferred.
  • Experience in health care preferred.
  • Experience with the geriatric population.
  • Experience working with people with Alzheimer's disease and related memory disorders preferred.
