About the position

The Social Services Director at Liberty Healthcare and Rehabilitation Services is responsible for coordinating social services for patients, ensuring compliance with established policies and regulations. This role involves leading social programs tailored to patient needs, facilitating admissions, and acting as a liaison between families and healthcare professionals. The director will also oversee documentation and quality assurance related to social work services.

Responsibilities

  • Serve as Social Services Coordinator for the facility.
  • Coordinate patient admissions in accordance with established policies and state/federal licensor requirements.
  • Ensure a meaningful social program appropriate to the needs of each patient is met.
  • Demonstrate leadership skills by planning and instituting new and innovative ideas and programs.
  • Receive inquiries of prospective residents and maintain records of inquiries.
  • Interview patients or family members during the admission application process.
  • Explain the cost of care involved in the appropriate level of care as determined by the admitting physician.
  • Act as a liaison between families, hospital discharge planners, social service agencies, and physicians.
  • Chair the Admission Conference/MDS meeting for new admissions.
  • Assist families or residents with discharge placement and discharge information as needed.
  • Monitor to assure appropriate sections on MDS are completed within 14 days of patient admission.
  • Monitor through Quality Assurance monitors per calendar.
  • Ensure that social work progress notes and electronic assessments are completed according to documentation policies.

Requirements

  • High school graduate with one year's experience in the health care or long-term care field, or an equivalent combination of education and experience.
  • Willingness to work and cooperate with other employees.
  • Ability to problem solve and work well under pressure.
  • Neat appearance and good hygiene.
  • Ability to read, know, and follow personnel, department, and facility policies.
  • Knowledgeable of local, state, and federal regulations related to the social needs of patients.
  • Valid N.C. driver's license.

Nice-to-haves

  • Desired LPN license.
