About the position

The Social Services Director at Northampton Manor Nursing and Rehabilitation Center is responsible for overseeing the social service programs within the facility. This role involves planning, developing, and implementing policies to address the social and emotional needs of patients and residents, ensuring they receive appropriate support and resources. The director will also collaborate with community agencies to facilitate discharge planning and provide referrals for specialized assistance.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the social service programs of the facility.
• Develop and implement policies and procedures for identifying the medically-related social and emotional needs of the patients/residents.
• Assist in providing solutions to social and practical environmental problems, including seeking financial assistance and discharge planning.
• Collaborate with community agencies and provide referrals for specialized assistance.

Requirements

• Must have a current Social Work license in good standing with the state.
• Minimum of one (1) year experience in a human services field in a hospital, skilled nursing care facility, or other related medical facility.
• Ability to organize and prioritize multiple tasks effectively.
• Skilled communicator, director, and motivator.
