About the position

The Social Services Director at Clark Manor is responsible for ensuring that residents receive high-quality social and emotional support during their stay. This role involves preparing residents for their future, coordinating discharges, and participating in care conferences to meet the residents' needs effectively. The position is integral to maintaining the standard of care expected at the facility and requires a compassionate approach to service delivery.

Responsibilities

• Prepare residents for their future by addressing social and emotional needs.
• Coordinate resident discharges to ensure smooth transitions.
• Participate in resident care conferences to discuss and plan for individual needs.
• Ensure that residents receive the high standard of care expected at Clark Manor.
• Provide support and guidance to residents and their families.

Requirements

• Bachelor's degree in Social Work or a similar field.
• Two years of experience in a Social Worker role, preferably in a healthcare setting.
• Prior experience as a Social Services Director in a Skilled Nursing Facility is preferred.
• Graduate degree in Social Work or a similar field is preferable but not mandatory.
• Five years of related experience is preferable.
