About the position

The Social Worker will be responsible for overseeing and assisting residents in a skilled nursing facility, focusing on their admission process, identifying needs, making referrals to community resources, and ensuring their mental and social health. This role involves developing relationships with residents and their families, assisting with care plans, and addressing any concerns that arise.

Responsibilities

  • Work with residents to oversee and assist in the admission process.
  • Identify needs and problems of residents and make referrals to community resources.
  • Ensure the mental and social health of each resident.
  • Develop relationships with residents and their responsible parties for admissions and counseling.
  • Assist residents with social interaction, reality orientation, and intellectual stimulation.
  • Help the Business Office Manager solve financial needs and promote emotional security for residents.
  • Complete acute care plans and attend care plan meetings as needed.
  • Assist the Administrator with managing resident/family/responsible party concerns.
  • Attend committee meetings as required and complete monthly QA reports.
  • Refer residents for vision, dental, and hearing services as needed.
  • Assist with room changes by contacting responsible parties and completing necessary forms.
  • Complete psychosocial and socialization assessments and refer residents for additional services if needed.
  • Discuss advance care planning with responsible parties upon admission and complete necessary documents.
  • Make morning rounds prior to daily meetings and provide staff with important updates.
  • Perform suicidal/homicidal assessments and follow up as required.
  • Assist in investigations as requested by the Administrator.
  • Complete the MDS as needed and work rotating weekend Manager on duty as scheduled.

Requirements

  • Bachelor's or Master's degree in Social Work.
  • Must be a Licensed Social Worker in the state of Texas.
  • Previous experience working in a skilled nursing facility is preferred.
