About the position

The Inpatient Social Worker Case Manager at AdventHealth Shawnee Mission plays a crucial role in providing comprehensive care to patients with complex psychosocial needs. This position is designed for a full-time schedule, operating during the day from Monday to Friday, with hours from 8 am to 4:30 pm. The Social Work Care Manager is responsible for assessing patients' psychosocial situations, determining eligibility for social programs, and facilitating transitions of care and discharge planning. This role requires collaboration with an interdisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure patient-centered care coordination throughout the continuum of care. The Social Work Care Manager intervenes with patients and families facing emotional, social, and financial challenges due to illness or disability. They provide crisis intervention, grief counseling, and education on available resources. The role also involves conducting psychosocial assessments, developing transition of care plans, and ensuring that patients receive the necessary support and services post-discharge. The Social Work Care Manager is expected to maintain effective communication with the healthcare team, participate in daily multidisciplinary rounds, and advocate for patient and family empowerment in healthcare decisions. In addition to direct patient care, the Social Work Care Manager is responsible for educating the healthcare team on resource utilization, medical necessity, and compliance with discharge planning regulations. They must stay informed about community resources and services available to patients, ensuring that care is efficient and cost-effective. The position requires a strong commitment to quality patient care, regulatory compliance, and outstanding customer service, all while promoting a collaborative and respectful environment within the healthcare team.

Responsibilities

  • Intervene with patients who have complex psychosocial needs.
  • Assist with eligibility determination for social programs and funding sources.
  • Provide crisis intervention to patients and families with psychosocial needs.
  • Coordinate and facilitate the development of a discharge plan of care for high-risk patient populations.
  • Ensure patient-centered care coordination through the continuum of care.
  • Monitor appropriate resources and escalate clinical care as needed.
  • Conduct patient evaluations of post-hospital needs and develop transition of care plans.
  • Communicate daily with the interdisciplinary team during multidisciplinary rounds.
  • Facilitate collaborative management of patient care across the continuum.
  • Educate nurses, physicians, and the interdisciplinary team on resource utilization and care coordination.

Requirements

  • Masters in Social Work (MSW)
  • Minimum three (3) years experience in hospital/medical social work
  • Care Management discharge planning experience
  • Knowledge of state and federal guidelines pertinent to care management
  • Excellent interpersonal communication and negotiation skills
  • Critical thinking and problem-solving skills

  • Ability to work with diverse social, economic, and cultural backgrounds
