About the position
The Social Work Care Manager at AdventHealth Riverview is responsible for addressing the complex psychosocial needs of patients. This role involves assisting patients with eligibility determination for social programs, coordinating discharge planning, and providing crisis intervention. The position is integral to the interdisciplinary team, ensuring that high-risk patient populations receive the necessary support and resources for successful transitions of care.
Responsibilities
- Intervene with patients who have complex psychosocial needs.
- Assist with eligibility determination for social programs and funding sources.
- Provide crisis intervention to patients and families with psychosocial needs.
- Coordinate and facilitate the development of a discharge plan of care for high-risk patient populations.
- Receive referrals for individuals from at-risk populations from interdisciplinary team members.
Requirements
- Experience in social work or case management.
- Knowledge of community resources and social programs.
- Ability to work collaboratively with interdisciplinary teams.
- Strong crisis intervention skills.
