About the position

The Social Work Care Manager PRN at AdventHealth Altamonte Springs is responsible for providing comprehensive psychosocial support to patients with complex needs. This role involves assessing patients' psychosocial risk factors, coordinating care transitions, and facilitating discharge planning to ensure efficient and effective patient care. The Social Work Care Manager collaborates with an interdisciplinary team to enhance patient flow, satisfaction, and safety while adhering to regulatory compliance and departmental policies.

Responsibilities

• Intervene with patients who have complex psychosocial needs and assist with eligibility determination for social programs.
• Provide crisis intervention to patients and families with psychosocial needs.
• Coordinate and facilitate the development of discharge plans for high-risk patient populations.
• Communicate daily with the interdisciplinary team during multidisciplinary rounds.
• Assess patients' and families' psychosocial risk factors and mobilize community resources to meet identified needs.
• Advocate for patient and family empowerment in healthcare decisions.
• Provide education to nurses, physicians, and the interdisciplinary team on resource utilization and care coordination.
• Participate in outstanding customer service and maintain respectful relationships with all stakeholders.

Requirements

• Masters in Social Work (MSW)
• Minimum three (3) years experience in hospital/medical social work

Nice-to-haves

• Care Management discharge planning experience
• Knowledge of state and federal guidelines pertinent to care management
• Licensed Clinical Social Worker (LCSW)
• ACM/CCM certification
