About the position
The Social Work Care Manager PRN at AdventHealth is responsible for providing comprehensive psychosocial support to patients with complex needs. This role involves assessing patients' psychosocial risk factors, coordinating care transitions, and facilitating discharge planning to ensure optimal patient flow and satisfaction. The Social Work Care Manager collaborates with an interdisciplinary team to deliver patient-centered care and educates team members on resource utilization and care coordination.
Responsibilities
- Intervene with patients who have complex psychosocial needs and assist with eligibility determination for social programs.
- Provide crisis intervention to patients and families with psychosocial needs.
- Coordinate and facilitate the development of discharge plans for high-risk patient populations.
- Communicate daily with the interdisciplinary team during multidisciplinary rounds.
- Conduct psychosocial assessments and interventions for patients and families.
- Advocate for patient and family empowerment in healthcare decisions.
- Provide grief counseling and crisis intervention skills.
- Educate patients, families, and caregivers regarding resource options and coping strategies.
- Assess and report interventions in cases of abuse or neglect as appropriate.
- Provide consult services for patients lacking decision-making capacity.
Requirements
- Masters in Social Work (MSW)
- Minimum three (3) years experience in hospital/medical social work.
Nice-to-haves
- Care Management discharge planning experience
- Knowledge of state and federal guidelines pertinent to care management
- Licensed Clinical Social Worker (LCSW)
- ACM/CCM certification
