About the position
The Social Work Care Manager at AdventHealth plays a crucial role in supporting patients with complex psychosocial needs. This position involves assisting patients in navigating social programs, funding sources, and community assistance, as well as providing crisis intervention and coordinating discharge planning. The Social Work Care Manager collaborates with an interdisciplinary team to ensure patient-centered care and efficient resource utilization, ultimately enhancing patient satisfaction and safety.
Responsibilities
- Intervene with patients who have complex psychosocial needs.
- Assist with eligibility determination for social programs and funding sources.
- Provide crisis intervention to patients and families with psychosocial needs.
- Coordinate and facilitate the development of discharge plans for high-risk patient populations.
- Communicate daily with the interdisciplinary team during multidisciplinary rounds.
- Educate nurses, physicians, and the interdisciplinary team on resource utilization and care coordination.
- Assess patients' psychosocial risk factors and mobilize community resources.
- Advocate for patient and family empowerment in healthcare decisions.
- Provide grief counseling and crisis intervention skills.
- Work in collaboration with hospital and community agencies to obtain needed services.
Requirements
- Masters in Social Work (MSW)
- Minimum three (3) years experience in hospital/medical social work.
Nice-to-haves
- Care Management discharge planning experience
- Knowledge of state and federal guidelines pertinent to care management
- Licensed Clinical Social Worker (LCSW)
- ACM/CCM certification
