About the position
The Social Work Care Manager is responsible for assessing patients and their families, arranging necessary interventions, and providing short-term treatment planning for various mental, emotional, and behavioral disorders. This role involves collaborating with healthcare teams, assisting with discharge planning, and identifying resources to support patients' needs. The position also includes supervising and training new staff and interns.
Responsibilities
- Assess patients and/or their families, caregivers, and legal representatives to arrange needed interventions.
- Plan interventions to help patients cope with social, emotional, economic, and environmental problems.
- Provide short-term treatment planning for mental, emotional, and behavioral disorders, conditions, and addictions.
- Participate in case reviews to evaluate care management and progress toward goals.
- Consult with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan.
- Assist with discharge planning and processes.
- Identify appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provide referrals as part of the discharge plan.
- Administer and interpret tests and measures of psychosocial functioning for diagnosing disorders and implementing treatment plans.
- Supervise and/or train new staff, lower-level social workers, students, or interns.
Requirements
- Master's Degree in Social Work from an accredited institution.
- Current State Licensure as a Licensed Clinical Social Worker (LCSW).
- Basic computer skills and knowledge of word processing and spreadsheet software.
- Strong written and verbal communication skills.
- Demonstrated ability to exercise critical thinking skills.
