About the position

The Clinical Social Worker, Licensed, plays a crucial role in addressing the complex psychosocial needs of patients within a healthcare setting. This position involves providing crisis intervention, coordinating care transitions, and ensuring patient-centered care through collaboration with an interdisciplinary team. The Clinical Social Worker is responsible for evaluating post-hospital needs, developing discharge plans, and facilitating access to community resources, all while promoting patient satisfaction and safety.

Responsibilities

  • Intervene with patients who have complex psychosocial needs.
  • Assist with eligibility determination for social programs and funding sources.
  • Provide crisis intervention to patients and families.
  • Coordinate and facilitate the development of discharge plans for high-risk patient populations.
  • Ensure patient-centered care coordination through the continuum of care.
  • Monitor resources and escalate clinical care as needed.
  • Communicate daily with the interdisciplinary team during multidisciplinary rounds.
  • Educate nurses, physicians, and the interdisciplinary team on resource utilization and care coordination.
  • Facilitate collaborative management of patient care across the continuum.

Requirements

  • Masters in social work (MSW)
  • Minimum five (5) years' experience in hospital/medical social work
  • Clinical Social Worker License (LCSW)
