About the position

The Social Work Care Manager at Intermountain Health is responsible for assessing patients and their families to arrange necessary interventions. This role involves planning interventions to assist patients in coping with various social, emotional, economic, and environmental challenges, as well as providing short-term treatment planning for mental and behavioral disorders. The position requires collaboration with healthcare teams to ensure compliance with treatment plans and effective discharge planning.

Responsibilities

  • Assess patients and/or their families, caregivers, and legal representatives to arrange for needed interventions.
  • Plan interventions to help patients cope with social, emotional, economic, and environmental problems.
  • Provide short-term treatment planning for mental, emotional, and behavioral disorders, conditions, and addictions.
  • Participate in case reviews to evaluate care management and progress toward goals.
  • Consult with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan.
  • Assist with discharge planning and processes.
  • Identify appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provide referrals as part of the discharge plan.
  • Administer and interpret tests and measures of psychosocial functioning for diagnosing disorders and implementing treatment plans.
  • Supervise and/or train new staff, lower-level social workers, students, or interns.

Requirements

  • Master's Degree in Social Work from an accredited institution.
  • Current State Licensure as a Licensed Clinical Social Worker (LCSW).
  • Basic computer skills and knowledge of word processing and spreadsheet software.
  • Strong written and verbal communication skills.
  • Demonstrated ability to exercise critical thinking skills.
