About the position

The Social Work Care Manager at Intermountain Wood Products is responsible for assessing patients and their families, arranging necessary interventions, and providing short-term treatment planning for various mental, emotional, and behavioral disorders. This role involves collaborating with healthcare teams, assisting with discharge planning, and identifying resources to support patients' needs.

Responsibilities

• Assess patients and/or their families, caregivers, and legal representatives to arrange needed interventions.
• Plan interventions to help patients cope with social, emotional, economic, and environmental problems.
• Provide short-term treatment planning for mental, emotional, and behavioral disorders, conditions, and addictions.
• Participate in case reviews to evaluate care management and progress toward goals.
• Consult with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan.
• Assist with discharge planning and processes.
• Identify appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provide referrals as part of the discharge plan.
• Administer and interpret tests and measures of psychosocial functioning for diagnosing disorders and disabilities.
• Supervise and/or train new staff, lower-level social workers, students, or interns.

Requirements

• Master's Degree in Social Work from an accredited institution.
• Current State Licensure as a Licensed Clinical Social Worker (LCSW).
• Basic computer skills and knowledge of word processing and spreadsheet software.
• Strong written and verbal communication skills.
• Demonstrated ability to exercise critical thinking skills.
