About the position

The Social Work Care Manager at UChicago Medicine AdventHealth plays a crucial role in supporting patients with complex psychosocial needs. This position involves assessing patients' needs, coordinating care, and facilitating discharge planning to ensure smooth transitions of care. The Social Work Care Manager collaborates with an interdisciplinary team to provide patient-centered care and resource management, ultimately enhancing patient satisfaction and safety while preventing readmissions.

Responsibilities

• Intervene with patients who have complex psychosocial needs and assist with eligibility determination for social programs.
• Provide crisis intervention to patients and families with psychosocial needs.
• Coordinate and facilitate the development of discharge plans for high-risk patient populations.
• Communicate daily with the interdisciplinary team during multidisciplinary rounds.
• Conduct psychosocial assessments and interventions for patients and families.
• Advocate for patient and family empowerment and independence in healthcare decisions.
• Provide education to patients, families, and caregivers regarding resource options and coping strategies.
• Collaborate with community agencies to obtain needed services for patients and families.
• Facilitate discussions regarding ethical dilemmas and advanced directives.
• Develop and document transition of care plans based on patient evaluations and preferences.
