About the position

The Clinical Social Worker, Licensed, plays a crucial role in addressing the complex psychosocial needs of patients within a healthcare setting. This position involves providing crisis intervention, coordinating care transitions, and ensuring patient-centered care through collaboration with an interdisciplinary team. The Clinical Social Worker is responsible for developing discharge plans, facilitating access to community resources, and enhancing patient flow and satisfaction while adhering to regulatory compliance and departmental goals.

Responsibilities

  • Intervene with patients who have complex psychosocial needs.
  • Assist with eligibility determination for social programs and funding sources.
  • Provide crisis intervention to patients and families with psychosocial needs.
  • Coordinate and facilitate the development of discharge plans for high-risk patient populations.
  • Receive referrals from interdisciplinary team members for at-risk individuals.
  • Ensure patient-centered care coordination through the continuum of care.
  • Monitor resources and escalate clinical care as needed for efficient and cost-effective care.
  • Conduct patient evaluations of post-hospital needs and develop transition of care plans.
  • Communicate daily with the interdisciplinary team during multidisciplinary rounds.
  • Educate nurses, physicians, and the interdisciplinary team on resource utilization and discharge planning.

Requirements

  • Masters in social work (MSW)
  • Minimum five (5) years' experience in hospital/medical social work
  • Clinical Social Worker License (LCSW)
