About the position
The Social Work Care Manager at AdventHealth Fish Memorial plays a crucial role in supporting patients with complex psychosocial needs. This position involves assessing patients' psychosocial risk factors, providing crisis intervention, and coordinating discharge planning for high-risk populations. The role emphasizes the importance of holistic care, addressing emotional, social, and financial challenges faced by patients and their families.
Responsibilities
- Intervene with patients who have complex psychosocial needs.
- Assist with eligibility determination for social programs and funding sources.
- Provide crisis intervention to patients and families with psychosocial needs.
- Coordinate and facilitate the development of a discharge plan of care for high-risk patient populations.
- Assess patient's and family's psychosocial risk factors through evaluation of prior functioning levels.
- Access and mobilize family/community resources to meet identified needs.
- Serve as a resource for treatment decisions, terminal illnesses, and end-of-life issues.
- Provide grief counseling and crisis intervention skills.
Requirements
- Masters in Social Work (MSW).
- Minimum three (3) years of experience in hospital/medical social work.
Nice-to-haves
- BLS Certification.
- Licensed Clinical Social Worker (LCSW).
- ACM/CCM certification.
- Care Management discharge planning experience.
- Knowledge of state and federal guidelines pertinent to care management.
