About the position

The Case Manager for Community Services at Gladden Community House plays a crucial role in providing direct services to clients in need. Under the supervision of the Vice President of Community Services, the Case Manager is responsible for delivering emergency provisional services and comprehensive case management to individuals and families facing basic needs challenges, such as rent and utility assistance. This position requires a compassionate and proactive approach to help clients navigate their immediate needs and connect them with appropriate resources. The Case Manager will engage in various activities, including screening and intake processes to assess clients' needs, as well as assisting them in completing applications for emergency financial assistance. This role involves short-term case management, where the Case Manager will refer clients to other human service agencies and coordinate with both formal and informal resource systems. The Case Manager will also be responsible for maintaining accurate documentation and database records of all client interactions and services provided, ensuring that all information is up-to-date and accessible. In addition to direct client services, the Case Manager will be expected to participate in training for the Alitta360 database, which is essential for entering and managing client information. The position is full-time, requiring an 8-hour shift from Monday to Friday, and is based in Columbus, OH. The Case Manager will work closely with various community organizations to ensure that clients receive the support they need to overcome their challenges and improve their overall well-being.

Responsibilities

  • Provide direct services to program clients, including emergency provisional services and case management.
  • Screen, refer, and provide short-term case management to clients seeking rent and utility assistance.
  • Complete Neighbor-to-Neighbor applications and Columbus Foundation 'Gifts of Kindness' applications.
  • Coordinate with formal resource systems, including other human service agencies.
  • Make additional resource referrals as needed to support clients.
  • Maintain accurate and up-to-date documentation and database records on all client services.
  • Enter client information into the Alitta360 database as required.

Requirements

  • Bachelor's degree in Social Work (B.S.W.) or a related field required.
  • One to two years of experience in basic needs case management services required.
  • Valid Ohio Driver's License required.
  • LSW (Licensed Social Worker) preferred.
