About the position

The Social Work Case Manager in Care Management is a professional clinician responsible for supporting patients and their families through care coordination. This role involves collaboration with an interdisciplinary team to advocate for patients, ensuring effective transitions of care and addressing social determinants of health to improve patient outcomes and experiences.

Responsibilities

• Contributes to and/or completes initial and ongoing comprehensive assessment, providing interventions and implementing recommendations after engaging patients and their caregivers/families.
• Collaborates to provide the safest transition plan for assigned patients (Inpatient/Observation/ED) to ensure timely discharge and connection with post-discharge care providers and community resources.
• Educates patients, families/caregivers, and physicians regarding the most appropriate level of care post-discharge and how to access community support, advocating for patients through effective communication with interdisciplinary team members.
• Serves as a resource for counseling and intervention related to treatment decisions and end-of-life issues, driving collaborative conversations to establish goals of care and providing crisis interventions in various sensitive situations.
• Promotes individual professional growth and development through certification, mentoring/precepting, and participation on department/hospital/system committees.
• Supports department-based goals that contribute to the success of the organization.

Requirements

• Master's degree in Social Work (to be obtained within 6 months of hire).
• Experience in a hospital or healthcare setting.
• LSW required within 1 year of hire; incumbents in the role on or before 12/31/2022 have until 12/31/2023 to obtain.
• Act 34 Criminal Background Clearance Certificate.
• Act 33 Child Abuse Clearance Certificate.
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
