About the position

The PRN Social Work Case Manager at Emory Healthcare is responsible for coordinating complex patient care needs across the healthcare continuum. This role involves assessing patients for transition needs, identifying those at risk for readmission, and facilitating high-quality clinical and cost outcomes. The Social Worker will collaborate with Utilization Review to address barriers to timely care delivery and reimbursement, ensuring that patients receive the necessary services and resources effectively and efficiently.

Responsibilities

  • Coordinate clinically complex patient care needs across the healthcare continuum.
  • Assess patients for transition needs and identify those at risk for readmission.
  • Facilitate high-quality clinical and cost outcomes.
  • Negotiate, procure, and coordinate services and resources needed by patients in a timely manner.
  • Collaborate with Utilization Review to identify and address barriers to care delivery and reimbursement.

Requirements

  • Master's degree in Social Work from an accredited institution.
  • At least 1 year of experience preferred, particularly in an Acute Care setting.
  • MSW license or license eligibility in the State of Georgia is required.
  • Knowledge of software applications is necessary.
  • ACM or CCM certification preferred.

Nice-to-haves

  • Experience in Behavioral Health settings.
  • License eligibility for LAPC/LPC in the state of Georgia.
