About the position

The Social Work Services Program Manager is responsible for overseeing the psychosocial, mental, and emotional needs of residents at the State Veterans' Home. This managerial role involves planning and implementing social services programs, developing policies, and coordinating case management and treatment services. The incumbent will also conduct assessments, provide training, and ensure compliance with standards of practice while working closely with interdisciplinary teams.

Responsibilities

  • Manage and coordinate case management, treatment, and counseling services for residents.
  • Conduct psychosocial assessments upon admission and annually thereafter.
  • Develop and implement policies and procedures for social services based on accepted standards of practice.
  • Provide training for professional staff on psychosocial intervention strategies.
  • Participate in interdisciplinary treatment team meetings to develop care plans.
  • Conduct group and individual therapy sessions as needed.
  • Lead monthly support groups and family council meetings.
  • Monitor participation of residents in programs designed to meet their psychosocial needs.
  • Perform off-site visits to veterans as requested by the NHA.
  • Assist in the selection and orientation of new personnel.

Requirements

  • Must be licensed as a Clinical Social Worker (LCSW) in the State of Florida.
  • Bachelor's degree in social work from an accredited institution.
  • Three years of professional social work experience (a master's degree can substitute for experience).
  • Proficient in Microsoft Office Suite and intermediate computer skills.
  • Exemplary documentation skills and excellent communication abilities.
  • Willingness to work weekends and holidays as needed.

Nice-to-haves

  • Experience in a long-term health care environment.
  • Knowledge of geriatric populations, including Alzheimer's Disease and PTSD.
  • Experience with crisis intervention and psychosocial assessments.
  • Familiarity with Minimum Data Set 3.0 and Baker Act procedures.
