About the position

The Social Work Specialist in the Complex Case Unit at Staffing Solutions Organization is responsible for providing support to Maine residents with disabilities facing complex healthcare, financial, and placement issues. This hybrid role involves facilitating team meetings, maintaining collaborative relationships with community agencies, and ensuring that members receive the necessary services in the least restrictive environment possible.

Responsibilities

• Provide solutions to complex healthcare, financial, and placement issues for residents with disabilities.
• Facilitate team meetings for adults without a discharge plan until a safe location is identified.
• Provide team meeting notes and update interventions in the CCU database.
• Consult and assist members in accessing needed services and resources.
• Partner with providers to develop a member plan of care.
• Maintain collaborative relationships with community agencies, hospitals, and providers.
• Assess agencies for services/waivers that members may be appropriate for.
• Perform other duties as assigned by the Department.

Requirements

• Six years of education and/or professional experience in social services casework or related fields.
• Licensed Social Worker (LSW) license issued by the Maine State Board of Social Worker Licensure.
• Knowledge of current standards of care and practices in healthcare services.
• Knowledge of applicable Department programs, policies, and guidelines.
• Knowledge of client rights and confidentiality.
• Knowledge of community resources and services.
• Ability to communicate and write clearly and effectively.
• Ability to collect and analyze casework material and make independent decisions.
• Ability to establish and maintain effective working relationships.
