About the position
The SW Care Manager Plus is responsible for coordinating patient care activities among various participants, including patients and their families, to ensure the effective delivery of healthcare services. This role involves collaboration with a multidisciplinary healthcare team to assess, evaluate, and develop action plans that address gaps in care, utilizing graduate-level clinical social work skills.
Responsibilities
- Systematically collects comprehensive and focused data relating to health needs and concerns of patients.
- Analyzes assessment data to determine patient/family/caregiver needs and facilitate appropriate services across the care continuum.
- Develops patient- and/or population-centered care plans that advocate for strategies and alternatives to attain expected outcomes.
- Implements identified patient/family-centered care plans to achieve expected outcomes.
- Employs educational strategies for patients, families, caregivers, and healthcare team members regarding treatment options and community resources.
- Evaluates the status and progress of patients toward expected outcomes and communicates this to relevant professionals.
Requirements
- Master's Degree in Social Work from an accredited program with a current unrestricted license as a Licensed Social Worker (LSW) in Ohio.
- A minimum of 3 years of clinical experience in healthcare is required.
- Experience with healthcare coordination in a managed care environment, homecare, or community agency is preferred.
- Strong communication (verbal and written), interpersonal, organizational, and leadership skills are required.
- Proficiency in MS Office (Outlook, Excel, Word) or similar software is required.
Nice-to-haves
- Pediatric ambulatory, care management, and/or health plan experience is preferred.
- Certification by a nationally recognized care management organization is preferred.
- Experience with Epic software or similar EMR software is preferred.
