About The Position

  • Ability to work with Veterans from diverse backgrounds.
  • Knowledge of medical and mental health diagnoses and treatment procedures.

Responsibilities

  • Provides case management and care coordination to facilitate appropriate delivery of health care services.
  • Links Veterans with services, resources, and opportunities to maximize their independence, health, and well-being.
  • Conducts timely assessments of Veterans in crisis to identify immediate needs and evaluate risk.
  • Provides interventions independently with Veterans and their families/caregivers experiencing complicated problems.
  • Formulates and implements treatment plans with measurable, achievable goals.
  • Serves on committees, work groups, and task forces as approved by the supervisor.
  • Provides subject matter consultation to colleagues and students on psychosocial treatment of Veterans.
  • Establishes and maintains ongoing education programs for Veterans and community agencies.
  • Assesses and documents behaviors or symptoms of abuse, neglect, or exploitation.
  • Collaborates with interdisciplinary teams to develop discharge plans and set achievable goals.
