About the position

The Social Worker Behavioral Health Interdisciplinary Program (BHIP) Care Coordinator position at the VA Northeast Ohio Healthcare System involves providing expert clinical and care coordination services to Veterans in an interdisciplinary outpatient mental health clinic. The role focuses on utilizing evidence-based interventions to address a wide range of mental and behavioral health conditions, ensuring comprehensive and seamless mental health services for Veterans.

Responsibilities

  • Provides expert clinical and care coordination services to Veterans served by the VA Medical Center's interdisciplinary outpatient mental health clinic.
  • Coordinates care for Veterans requiring higher levels of mental health specialty care, including biopsychosocial assessments and crisis services.
  • Acts as an advocate for Veterans on BHIP teams to ensure comprehensive mental health service delivery.
  • Participates in program planning and development, advising on care coordination needs and recommending policy changes.
  • Conducts biopsychosocial assessments to determine care coordination and treatment needs of Veterans.
  • Provides crisis intervention in collaboration with the multidisciplinary treatment team.
  • Educates Veterans and families on VA resources, community resources, and options for community care.
  • Participates in transition planning to ensure timely and appropriate execution of care continuity.

Requirements

  • Must be a U.S. Citizen.
  • Master's degree in social work from an accredited school of social work.
  • Licensed or certified by a state to independently practice social work at the master's degree level.
  • Proficient in written and spoken English.
  • Ability to work with diverse populations and assess psychosocial functioning.
  • Knowledge of medical and mental health diagnoses and treatment procedures.
