Responsibilities

  • Monitor and evaluate the success of care plans and report outcomes.
  • Develop strategies to coordinate post-acute and long-term care services to prevent exacerbations.
  • Perform initial and periodic assessments of members enrolled in care programs.
  • Apply social work concepts to address social determinants of health in care plans.
  • Conduct regular discussions with providers and care management staff regarding member progress.
  • Advocate for members to ensure they receive necessary care under their benefit plans.
  • Build relationships with hospital social workers and community resources for effective care management.
  • Identify and report potential quality improvement issues.
  • Direct social work interventions including psychosocial assessments and documentation.

Requirements

  • Minimum of 2+ years' experience in medical social work or case management.
  • Active and unrestricted LCSW or LMSW in NY.
  • Ability to travel 50-75% within the Buffalo area.
  • Master's degree in Social Work.
