About the position

Ennoble Care is seeking an experienced Full-time Social Worker Care Manager (Remote) for our hospice department within and around Philadelphia, PA region. This position will be a hybrid role with responsibilities that include supporting the mental and physical health care of patients on an assigned patient caseload, screening and assessing patients for common mental health and substance abuse disorders, and facilitating patient engagement and follow-up care. The Social Worker Care Manager will also provide patient education about treatment options, track treatment responses, support medication management, and document patient progress in the EHR system.

Responsibilities

  • Support the mental and physical health care of patients on an assigned patient caseload.
  • Screen and assess patients for common mental health and substance abuse disorders.
  • Facilitate patient engagement and follow-up care.
  • Provide patient education about common mental health and substance abuse disorders and the available treatment options.
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
  • Support psychotropic medication management by consulting with the behavioral health consultant and communicating with the primary care provider.
  • Provide brief behavioral interventions using evidence-based techniques.
  • Provide or facilitate referrals to evidence-based psychosocial treatments as clinically indicated.
  • Participate in regularly scheduled caseload consultations with the psychiatric consultant.
  • Track patient follow up and clinical outcomes using a registry.
  • Document patient progress and treatment recommendations in EHR and other required systems.
  • Facilitate treatment plan changes for patients not improving as expected.
  • Facilitate referrals for clinically indicated services outside of the organization.
  • Develop and complete relapse prevention self-management plans with patients.
  • Utilize Excel and Word for assessments and productivity trackers.
  • Manage time effectively through calendars and the EMR system.
  • Conduct both telephonic and in-person visits.

Requirements

  • Minimum of MSW.
  • Preferred Licensure or provisionally licensed.
  • Behavioral Health experience and knowledge.
  • Must be a licensed driver with an insured automobile in good working order.
  • Ability to communicate effectively, both verbally and in writing.
  • Time Management skills.
