About the position

The Care Management model provides effective transition planning and length of stay oversight while encompassing quality patient care, patient safety, and financial components; promotes integration of a seamless care model; coordinates patient throughput; encourages healthcare team collaboration and coordination as evidenced by metrics; and optimizes performance and adoption of best practice.

Responsibilities

  • Intervene with patients who have complex or high-risk psychosocial needs
  • Offer crisis intervention to patients and families
  • Coordinate and facilitate the development of a discharge plan of care
  • Receive referrals for individuals from at-risk populations from interdisciplinary team members
  • Perform duties and responsibilities in a manner consistent with the Mercy mission, values, and Service Standards

Requirements

  • Education: Master's in Social Work
  • Licensure: Current license in Social Work in the state of employment (LMSW, LCSW)
  • Experience: 0-2 years experience in acute care hospital setting
  • Certification/Registration: BLS (CPR) at hire date, required, or within 90 days of hire
  • Strong verbal and written communication and facilitation skills
  • Strong assessment skills, time management skills, critical thinking skills
  • Advanced computer skills
  • Ability to care for patient age groups including neonatal/infant, child, adolescent, adult, and geriatric adult patients
  • Skills and knowledge of human behavior and clinical aspects of medical Social Work
  • Ability to interact effectively with a variety of people and situations at all levels of the organization
