About the position

The Social Worker, Care Manager Relief at AdventHealth Shawnee Mission is responsible for providing comprehensive psychosocial support to patients with complex needs. This role involves assessing patients' psychosocial situations, coordinating care transitions, and ensuring effective discharge planning. The Social Work Care Manager collaborates with an interdisciplinary team to facilitate patient-centered care and resource management, ultimately enhancing patient satisfaction and safety.

Responsibilities

• Intervene with patients who have complex psychosocial needs.
• Assist with eligibility determination for social programs and funding sources.
• Provide crisis intervention to patients and families with psychosocial needs.
• Coordinate and facilitate the development of discharge plans for high-risk patient populations.
• Communicate daily with the interdisciplinary team during multidisciplinary rounds.
• Educate nurses, physicians, and the interdisciplinary team on resource utilization and care coordination.
• Conduct psychosocial assessments and interventions for patients and families.
• Advocate for patient and family empowerment in healthcare decisions.
• Facilitate patient care conferences with the multidisciplinary team as needed.
• Ensure compliance with regulatory requirements and quality patient care.

Requirements

• Master's degree in Social Work (MSW).
• Licensed Clinical Social Worker (LCSW) certification.
• Minimum of three years of experience in hospital/medical social work.
• Experience in care management and discharge planning.
• Knowledge of state and federal guidelines pertinent to care management.

Nice-to-haves

• Certified Case Manager (CCM) certification.
• Experience in post-acute care settings.
• Strong leadership and data analysis skills.
