About the position

The Inpatient Social Worker Case Manager at AdventHealth Shawnee Mission plays a crucial role in providing comprehensive psychosocial support to patients with complex needs. This position involves assessing patients' psychosocial risk factors, coordinating care transitions, and facilitating discharge planning to ensure optimal patient outcomes. The Social Work Care Manager collaborates with an interdisciplinary team to deliver patient-centered care, addressing emotional, social, and financial challenges faced by patients and their families. The role emphasizes crisis intervention, resource mobilization, and education for both patients and healthcare providers, ensuring a smooth transition from hospital to home or other care settings.

Responsibilities

  • Intervene with patients who have complex psychosocial needs and assist with eligibility determination for social programs.
  • Provide crisis intervention to patients and families with psychosocial needs.
  • Coordinate and facilitate the development of discharge plans for high-risk patient populations.
  • Communicate daily with the interdisciplinary team during multidisciplinary rounds.
  • Conduct psychosocial assessments and interventions for patients and families.
  • Advocate for patient and family empowerment in healthcare decisions.
  • Provide education to nurses, physicians, and the interdisciplinary team on care coordination and resource utilization.
  • Develop and implement transition of care plans prior to patient discharge.
  • Monitor patient flow and ensure continuity of care to enhance patient satisfaction and safety.

Requirements

  • Masters in Social Work (MSW)
  • Minimum three (3) years experience in hospital/medical social work
  • Care Management discharge planning experience
  • Licensed Clinical Social Worker (LCSW)
  • ACM/CCM certification
  • Excellent interpersonal communication and negotiation skills
  • Critical thinking and problem-solving skills
  • Ability to work with diverse populations and adapt to change.
