The Social Work Care Manager plays a crucial role in supporting patients with complex psychosocial needs at AdventHealth Ocala. This position focuses on assisting patients in navigating social programs, funding sources, and community assistance, while also providing crisis intervention and coordinating discharge planning. The role emphasizes patient-centered care and collaboration with an interdisciplinary team to ensure efficient and effective care delivery.

Responsibilities

• Intervene with patients who have complex psychosocial needs.
• Assist with eligibility determination for social programs and funding sources.
• Provide crisis intervention to patients and families with psychosocial needs.
• Coordinate and facilitate the development of discharge plans for high-risk patient populations.
• Receive referrals from interdisciplinary team members for at-risk individuals.
• Ensure patient-centered care coordination through the continuum of care.
• Collaborate with care manager nurses, physicians, and other team members to ensure efficient and cost-effective care.

Requirements

• Experience in social work or a related field.
• Knowledge of community resources and social programs.
• Strong communication and interpersonal skills.
• Ability to work collaboratively in an interdisciplinary team.
