About the position

The Social Worker - Case Management position at St. Joseph's Medical Center involves coordinating patient care from pre-admission through hospitalization to post-discharge follow-up. Responsibilities

  • Coordinate patient care from pre-admission through hospitalization to post-discharge follow-up.
  • Build collaborative relationships with patients, families, clinical nurses, physicians, and other health team members.
  • Assist patients and families in coping with problems resulting from severe or long-term illness.
  • Conduct interviews to obtain psycho-social history from patients and families.
  • Resolve environmental difficulties that interfere with medical care benefits.
  • Prepare and update social work case records, documenting social problems and patient progress.
  • Direct patients to community agencies or resources as necessary.
  • Utilize a problem-solving process including assessment, problem identification, goal definition, plan development, evaluation, and revision.

Requirements

  • Master's degree in Social Work or current state license as a Social Worker or Professional Counselor required.
  • Current American Heart Association Healthcare Provider certification required.
  • Knowledge of social work theories, therapies, and techniques of casework processes.
  • Approximately six to twelve months of job experience in discharge planning and knowledge of health and welfare resources.

Nice-to-haves

  • Coursework in Medical Social Work preferred from an accredited Graduate School of Social Work.
  • Experience in healthcare settings.
