About the position

The Service Coordinator II position at Tri-Counties Regional Center involves providing person-centered consultation, advocacy, resource information, and service coordination for individuals with developmental disabilities. The role requires maintaining regular contact with clients, conducting assessments, and coordinating services to support their needs effectively within a team environment.

Responsibilities

  • Support individuals by maintaining regular contact and conducting Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
  • Participate in Interdisciplinary Team (IDT) staffings and attend community meetings, including Individual Education Plan (IEP) and day program meetings.
  • Provide information and referrals to community services and resources, including schools, recreational options, and local support groups.
  • Coordinate and secure TCRC funded services such as behavior services, independent living training, and respite care.
  • Conduct strength-based assessments to understand the unique issues facing family members.
  • Involve individuals and families in person-centered planning and support activities.
  • Design, document, implement, and monitor services and supports in a timely manner.
  • Facilitate planning team progress, identifying outcomes and needed services.
  • Develop and maintain a network of vendors and community agencies.
  • Provide training and information about developmental disabilities and available resources.

Requirements

  • Bachelor's degree in a Human Services or related field.
  • Valid California driver's license and transportation or acceptable substitute.
  • Knowledge of developmental disabilities and public benefit programs.
  • Problem-solving ability and organizational skills.
  • Ability to make verbal and written presentations to groups.
  • Experience working with individuals with developmental disabilities preferred.

Nice-to-haves

  • Master's degree in a relevant field.
  • Experience in time management and effective communication skills.
