About the position

The Social Worker/Case Manager position at YAI involves supporting individuals with intellectual and developmental disabilities (I/DD) in a group home setting. The role focuses on assisting with the placement of clients into care homes, developing and implementing individualized service plans, and ensuring compliance with relevant regulations. The position requires collaboration with various stakeholders to promote the well-being and successful transition of clients into long-term living arrangements.

Responsibilities

  • Assist in the completion of intake studies, needs and services plans, and discharge plans for each person in the home.
  • Participate in the transitional planning process, including IEP, IPP, and IBSP meetings, and seek out community resources to support individual needs.
  • Ensure that each individual's Needs and Services Plan is regularly reviewed and updated, at least every six months.
  • Act as a liaison between the home and various parties, including family members and social services agencies.
  • Maintain the persons' services records per California statute and regulations.
  • Provide training for group home staff regarding support needs and service plans.
  • Conduct monthly Residential Council Meetings with residents and update relevant staff on proposed activities.
  • Attend and participate in meetings related to the service and support needs of residents.
  • Provide monthly recommendations and document using a digital platform.
  • Complete certified trainings such as CPR/FA and Ukeru.
  • Collaborate with the RN, QBMP, and Administrator in supporting residents and managing schedules.
  • Build and maintain positive relationships with staff and residents.
  • Assist with the development of new residential programs.
  • Perform all other duties as assigned.

Requirements

  • Bachelor's Degree in Social Work or a related field with at least three years of social work experience under supervision.
  • Knowledge of California's Lanterman Act and related service systems, Title 17 and/or Title 22 regulations.
  • Ability to work in a stressful environment and manage multiple priorities.
  • Exceptional verbal and written communication skills.
  • Proficiency in technical systems such as Behavior Agent, WorkDay, and Microsoft Teams.
  • Ability to perform physical tasks including lifting up to 25 pounds and standing for extended periods.

Nice-to-haves

  • Master's Degree in Social Work or a related field.
  • Knowledge of the unique needs of individuals with I/DD.
  • Familiarity with residential services for people with I/DD.
