About the position

The Medical Social Worker in Home Health at CenterWell plays a crucial role in providing interdisciplinary care to home health patients. This position involves evaluating and developing personalized care plans that address the emotional and social needs of patients. The Medical Social Worker also supervises Social Worker Assistants and ensures compliance with company policies and healthcare standards.

Responsibilities

  • Assess the patient's social and emotional state related to their illness or injury and their response to treatment.
  • Evaluate the patient's medical and nursing needs in the home setting, including financial resources and community resources.
  • Facilitate access to community resources to assist in resolving issues that may impede the patient's recovery.
  • Instruct patients and families on coping with social and emotional responses related to their health conditions.
  • Provide ongoing assessment of patient and family needs and responses to teaching.
  • Assist the healthcare team in understanding significant social and emotional factors affecting the patient's health.
  • Participate in the development and periodic re-evaluation of the physician's Plan of Care for the patient.
  • Observe, record, and report changes in patients' conditions and responses to treatment to the Clinical Manager.
  • Participate in discharge planning processes as a member of the interdisciplinary care team.
  • Supervise and evaluate the performance of the Social Work Assistant to ensure compliance with standards.
  • Maintain and submit required documentation, including case conferences and visit reports.

Requirements

  • Masters or doctoral degree from an accredited school of social work.
  • Social Worker licensure in the state of practice, if required by state law.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Minimum of one year of experience as a social worker in a healthcare setting, home health, or hospice.
  • Knowledge of discharge planning needs and community resources.
  • Excellent oral and written communication and interpersonal skills.
