About the position

The Medical Social Worker plays a crucial role in providing interdisciplinary care to home health patients, focusing on their emotional and social needs. This position involves evaluating patients, developing personalized care plans, and supervising Social Worker Assistants. The Medical Social Worker collaborates with healthcare teams to address the social and emotional factors affecting patients' health and assists in discharge planning and resource allocation.

Responsibilities

• Evaluate and develop personalized care plans for patients based on their emotional and social needs.
• Supervise and provide direction to Social Worker Assistants as required.
• Assess patients' social and emotional states in relation to their illness or injury.
• Identify and coordinate community resources to assist patients in their recovery.
• Instruct patients and families on coping with social and emotional responses related to health issues.
• Participate in the development and re-evaluation of the physician's Plan of Care.
• Observe, record, and report changes in patients' conditions to the Clinical Manager.
• Participate in discharge planning and care coordination activities.
• Maintain and submit required documentation, including progress notes and visit reports.

Requirements

• Master's or doctoral degree from an accredited school of social work.
• Social Worker licensure in the state of practice, if required by state law.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Minimum of one year of experience as a social worker in a healthcare setting, home health, or hospice.
• Knowledge of discharge planning needs and community resources.
• Excellent oral and written communication and interpersonal skills.
