About The Position

The Social Worker position at Blossom Ridge Home Health Agency involves providing medical social services to patients under the direction of a physician and an interdisciplinary team. The role focuses on understanding the social and emotional factors affecting patients' health, developing coping mechanisms, and assisting patients and their families in navigating medical recommendations and community resources. The Social Worker will also mentor and supervise other social workers, contribute to policy development, and facilitate caregiver support groups.

Requirements

  • Master's Degree from a school of Social Work accredited by the Council of Social Work Education.
  • One year of social work experience in a health care setting.
  • Current CPR certification required.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

  • Provides medical social services under the direction of a physician and Interdisciplinary Group.
  • Assesses social and emotional factors related to patient's illness to determine ability to cope with daily living problems.
  • Assists patients and significant others in understanding and following medical recommendations.
  • Identifies gaps in community resources and stimulates resource development as needed.
  • Visits patients according to the Plan of Care and completes clinical notes for each visit.
  • Functions as Bereavement Coordinator and supervises bereavement services as needed.
  • Contributes to the development of a comprehensive, integrated Plan of Care for patients.
  • Instructs health care team members on community resources available to assist patients.
  • Collaborates with hospitals and community agencies to promote continuity of care.
  • Mentors new hire social workers through orientation and ongoing education.
  • Facilitates monthly caregiver support groups.
