About the position

The Service Coordinator II Early Childhood position at Tri-Counties Regional Center involves providing person-centered consultation, advocacy, and service coordination for individuals with developmental disabilities. This role requires bilingual proficiency in English and Spanish and focuses on maintaining regular contact with clients, conducting assessments, and coordinating services to support their needs effectively.

Responsibilities

  • Support individuals by maintaining regular contact and conducting Individual Family Service Plans (IFSP) and Individual Program Plans (IPP).
  • Participate in Interdisciplinary Team (IDT) staffings and attend community meetings, including Individual Education Plan (IEP) and day program meetings.
  • Provide information and referrals to community services and resources, including schools, recreational options, and local support groups.
  • Coordinate and secure TCRC funded services such as behavior services, independent living training, and respite care.
  • Conduct strength-based assessments to understand the unique issues facing families.
  • Involve individuals and families in all aspects of person-centered planning and support activities.
  • Design, document, implement, and monitor services and supports in a timely manner.
  • Facilitate planning team progress, identifying outcomes, needed services, and supports.
  • Develop and maintain a network of vendors and community agencies.
  • Provide relevant information and training about developmental disabilities, services, and resources.
  • Use effective communication and interpersonal skills to respect individuals with developmental disabilities and their families.
  • Demonstrate knowledge of family support and integrate these into service planning for community-based outcomes.
  • Demonstrate knowledge about developmental disabilities and their impact on families.
  • Demonstrate knowledge of various intervention and support approaches.
  • Demonstrate knowledge of entitlement and benefit programs.
  • Demonstrate knowledge of the Tri-Counties Regional Center system and advocacy.
  • Work cooperatively as a team member and maintain good relationships with community representatives.
  • Participate in professional growth and development through training and conferences.
  • Conduct all activities in a professional and ethical manner.

Requirements

  • Bachelor's degree in a Human Services or related field.
  • Minimum of 2 years of social and/or mental health experience.
  • Valid California driver's license and transportation or acceptable substitute.
  • Bilingual ability in Spanish and English preferred.
  • Experience working with individuals with developmental disabilities preferred but not required.
