About the position

The individual in this position is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and interventions to promote timely throughput, safe discharge and prevent avoidable readmissions. This position integrates national standards for case management scope of services including:

Responsibilities

  • Facilitate care along a continuum through effective resource coordination.
  • Assess patients for transition needs and identify those at risk for readmission.
  • Conduct complex psycho-social assessments and interventions.
  • Promote timely throughput and safe discharge to prevent avoidable readmissions.
  • Manage transitions to promote appropriate length of stay and patient satisfaction.
  • Ensure compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy.
  • Provide education to physicians, patients, families, and caregivers.
  • Participate in planning the social work component required in selected hospital programs.
  • Provide in-service education for staff.
  • Collaborate with community providers to develop educational resources for staff and patients.

Requirements

  • Experience in case management or related field.
  • Strong assessment and intervention skills.
  • Knowledge of regulatory requirements and accreditation standards.
  • Ability to collaborate effectively with healthcare providers and community resources.

Nice-to-haves

  • Certification in case management or related specialty.
  • Experience in a healthcare setting, particularly in acute care.
