Responsibilities

  • Provide complex and specialized social work consultation and comprehensive case management services to families.
  • Conduct home visits for comprehensive assessment of home environment and needs.
  • Assist families with the eligibility determination process for various health programs.
  • Maintain documentation of work activities, including monthly reports and case summaries.
  • Conduct comprehensive assessments to determine eligibility and need for personal care services.
  • Provide consultation, presentations, and training to service providers and contractors.
  • Participate in community organization functions and collaborate with other entities on health and social work issues.
  • Attend trainings and conferences relevant to the job to meet licensing requirements.
  • Participate in disaster response and recovery efforts as needed.

Requirements

  • Current, unrestricted license to practice social work in the State of Texas.
  • Bachelor's or graduate degree from a Council on Social Work Education accredited college or university.
  • Knowledge of Case Management principles and practices.
  • Knowledge of children's programs such as Medicaid, CSHCN, and CPS/Foster Care.
  • Skill in using computer software programs including MS Word, PowerPoint, Excel, Access, and Outlook.
  • Ability to work effectively as a team member in an interdisciplinary setting.

Nice-to-haves

  • Master's Degree preferred.
  • Bilingual (Spanish) preferred.
