About The Position

Responsibilities

• Conduct initial assessments and reassessments for In-Home Supportive Services according to regulations.
• Develop and implement case plans for assigned caseloads, ensuring services are delivered respectfully and appropriately.
• Maintain case records and communicate decisions and service authorizations in a timely manner.
• Provide counseling and support to clients with complex needs, including crisis intervention.
• Refer clients to county and community resources and advocate for appropriate services.
