The Social Worker will work within a multidisciplinary team to ensure comprehensive care delivery.

Responsibilities

• Establish working relationships of mutual respect and trust with patients and families.
• Utilize various helping interventions to enable patients to function effectively.
• Identify and evaluate clients' or patients' problems and needs.
• Develop service or treatment plans in collaboration with other professionals.
• Initiate and implement service plans, either directly or through authorized services.
• Monitor the services being provided and assess clients' progress.
• Make amendments to service/treatment plans as necessary.

Requirements

• Bachelor's or master's degree in social work from an accredited program.
• Progressively responsible professional work experience in social work (2 years for BSW, 1 year for MSW).
• Experience in helping individuals and families identify problems and cope with conditions.
