About the position

The Social Worker LCSW provides essential services to patients, residents, and their families or caregivers, addressing their social and emotional needs in relation to medical diagnoses and concerns. This role involves collaboration with a multidisciplinary team to ensure that patients receive comprehensive treatment, recovery, and therapeutic services, facilitating safe and coordinated transitions between different care environments. The Social Worker acts as an advocate for the rights of patients and residents, striving to ensure that each individual achieves the highest practicable level of physical, mental, and psychosocial well-being. Working under the direction of Care Management Leadership, the Social Worker LCSW is employed by Providence Health & Services and practices in accordance with the competencies and guidelines outlined in the Standards for Social Work Practice in Health Care Settings, as well as the Code of Ethics defined by the National Association of Social Workers. This position is integral to the patient care process, ensuring that the social aspects of health care are addressed alongside medical treatment. Requirements

  • Master's Degree in Social Work from an accredited School of Social Work.
  • Alaska Clinical Social Worker License upon hire.

Nice-to-haves

  • 2 years of Medical Social Work experience.
  • Experience with computerized documentation and medical records.
